Earlier in the month I sent out a Teacher Handbook for Transition from
High School to Higher Education for evaluation for a graduate project. 
If you have the your evaluation completed please sent it with the
enclosed envelope or fax it at 801-626-8990 by April 25 if possible.  If
you didn't receive this mailing, they are attached in this email-one is
the handbook and other is the evaluation.

Your parcipation to this project is very important for its success.
