[Kan-ahead] Tax Forms

For those of you that pay note takers  -- does your school provide an W-9 tax form for independent contractors, or any information about taxation? I've been perusing the IRS web site and I cannot find a "minimum amount earned" to require having to file the form. Hoping someone else has already figure this out....?!
